General Information:
This employee ensures the provision of Assertive Community Treatment (ACT) fidelity model of care to adults with severe and persistent mental illness and substance use challenges in the Middle Peninsula Northern Neck Community Services Board (MPNN CSB) service area via care coordination, rapport building, facilitating meeting basic needs, motivational interviewing techniques, skill restoration and development, and crisis treatment.  Will be required to transport persons served in the MPNN CSB vehicle. Delivers all services according to a recovery-based philosophy of care.  Must be able to work a flexible schedule, including some evenings, weekends, and holidays; participation in on-call rotation.  Provides all services in the community as well as individual’s homes. This position works closely with other team members using a team approach. Critical component of job is to possess strong interpersonal skills. Employee must exercise tact and courtesy in contact with staff, Individuals, families, public, and service providers. 

Minimum Training and Experience:

Bachelor or Master’s degree in human services field (i.e. Psychology, Sociology, Social Work, or Counseling). Required Qualified Mental Health Professional (QMHP) OR Qualified Mental Health Professional-Trainee (QMHP-E).

Special Requirements:
Must have a TB test with results available within 30 days of orientation and yearly as required.
