General Information:

General Information: Position Available in Gloucester and Warsaw

Employee provides general education on chronic diseases to individuals with serious mental illness and staff.  Assists individuals in identifying health conditions, assess their needs and abilities to manage their illness, an identify barriers to access health care. Incumbent may provide nursing care and medication management in an outpatient setting.  Works collaboratively with other team members to provide education for individuals with high health risks. Work is performed under general supervision and is guided by agency policies and procedures, quality assurance, public and private payor guidelines, professional practices and including ethical standards related to client interactions, Board of Pharmacy and Board of Nursing regulations as appropriate, state and federal licensure and accreditation regulations and guidelines. Teaches patients and their families about diagnosed illnesses, medications, and treatments by providing them with information and skills to better manage their conditions.



Minimum Training and Experience:
Bachelor's (or Associate’s) Degree in Nursing and 1-2 years professional expe­rience in psychiatric nursing serving adults with mental illness, emotionally disabled and/or individuals with substance use disorders, preferred. Professional experience in patient education.  Master’s Degree in Public Health may be considered.

Special Requirements:

State Certified Registered Nurse preferred, but not required. State Certified LPN minimum requirement. Must possess a valid Virginia Driver’s License.  Must obtain a TB test with satisfactory results by first day of employment and annually thereafter as required by licensing authority.
