Transitional Care Case Manager

General Information:
Under varying levels of supervision, employee is responsible for providing Crisis Stabilization services and securing benefits and entitlements for persons served which may have cognitive, mental health and/or substance abuse challenges. Some additional short term Case Management Services to assist individuals in accessing services and supports will also be provided. Employee is responsible for developing and maintaining effective working relationships with community agencies and resources.    Must have working knowledge of Federal and State public assistance programs.

Minimum Training and Experience:

Bachelor’s Degree in Sociology, Social Services, Psychology, or related field, and 1 years experience required in Case Management with some significant experience in  benefits/insurance/entitlements. Masters degree preferred.  Social Services and/or Social Security Administration experience strongly preferred.  

Special Requirements:

Must possess a valid Virginia Driver's License.  Must obtain a TB test with satisfactory results by orientation.  Must maintain current CPR and First Aid. Must receive TOVA, MANDT or other comparable non violent crisis intervention training within six months of hire.
