Adult Mental Health Case Manager

General Information:
Employee is responsible for providing case management services and crisis intervention to mental health and substance abuse clients.  Case management services include hospital discharge planning; assisting clients in identifying employment opportunities, living alternatives, educational, and entitlement opportunities.  Employee provides crisis intervention on an on-call basis as assigned by the Director of Clinical Services.  Employee acts as a liaison between the Counseling Center and psychiatric institutions (both public and private) and hospitals, relaying appropriate clinical information. Employee must exercise tact and courtesy in contact with staff, clients, families, public, and service providers. 

Minimum Training and Experience:

BA/BS in Human Services, Social Work, Psychology, Sociology or related field, and 1 to 2 years experience desired in emergency/crisis services or case management; or any equivalent combination of training and experience which provides the required knowledge, skills, and abilities.


Special Requirements:

Must possess a valid Virginia Driver's License.  Must obtain a TB test with satisfactory results by first day of employment and annually thereafter as required by licensing authority.
