Substance Use Case Manager

General Information:
Employee is responsible for providing case management services to pregnant women and women with dependent children who are receiving substance abuse services.  Case management services include but are not limited to hospital discharge planning; assisting clients in identifying employment opportunities, living alternatives, educational, and entitlement opportunities as well as childcare options, and birthing plans.  Employee acts as a liaison between the Counseling Center and maternity wards, OBGYN clinics, pediatric clinics and hospitals, relaying appropriate clinical information. Employee must exercise tact and courtesy in contact with staff, clients, families, public, and service providers. 

Minimum Training and Experience:

BA/BS in Human Services, Social Work, Psychology, Sociology or related field, and has at least one year of substance abuse related clinical experience or BA/BS in any field with certification as a substance abuse counselor or certified addictions counselor.

Special Requirements:

Must possess a valid Virginia Driver's License.  Must obtain a TB test with satisfactory results within 30 days of employment and annually thereafter as required by licensing authority.
