Senior Support Coordinator

General Information:

Under general supervision, employee provides assistance to the Support Coordinator Supervisor in the daily operations.  This position will provide daily supervision to an assigned team of Support Coordinators. Primary duties:

Develops and coordinates Person Centered Plans and case management service for a limited caseload; oversees the development of behavioral and vocational programs, and skill training programs for individuals; acts as an advocate for the individual in the community and in dealings with outside agencies.


Monitors individual’s satisfaction with services and makes modifications to existent program plans as needed; preforms assessments of individuals to determine training needs; monitors residential and day support services; and maintains case management records in accordance with regulations and standards.


Assists in procuring needed services and resources for individuals; monitors residential placement program; monitors day support program; monitors medication records; arranges medical appointments; oversees the management of individual’s personal funds.


Minimum Training and Experience:


Bachelor's Degree required, with Masters preferred in social work, case management, intellectual disabilities health assessment, or related field, with a minimum of 3 years’ experience working with individuals with intellectual disabilities. Experience as a supervisor preferred. 


Special Requirements:

Must possess a valid Virginia Driver's License and driving record acceptable to agency insurance carrier.  Must possess CPR and First Aid Certification.  Must be certified tuberculosis free by day of orientation and tested annually as required by licensing authority.
