Family Support Worker/Case Manager

General Information:
Under direct supervision, employee provides intensive home visiting services, using the Healthy Families model, to families at-risk for child abuse and neglect, poor health, and poor child developmental outcomes. The services provided will be family centered and strength based. The worker will initiate and maintain regular (weekly) and long-term (up to 5 years) supportive contact with families. Transportation or accompanying families for regular health care and other business-related appointments may be required. Responsible for documenting family activity and progress, coordinating with other agencies and organizations, participate in structured in-service trainings and supervision, and participate in regular scheduled staff meetings.

Minimum Training and Experience:
Applicant must have at least a Bachelor's degree in Human Services, Nursing (or related field), and experience working with prenatal families and young children.

Special Requirements:
Must possess a valid Virginia Drivers License. Must obtain a TB test with satisfactory results by first day of employment. Position open until filled. EOE
