GENERAL STATEMENT OF JOB

 

Under limited supervision, employee is responsible for providing assessments and reassessments for adults who are residents of Tidewater Cove Adult Living Facility, Suffolk, VA and who have been referred for skill building services. The assessments will include a social history and will also include an evaluation of the individual’s:

 

  • Functional skills to include appropriate behavior related to the individual’s health and safety,
  • Activities of daily living to include the use of community resources,
  • Ability to assist with their medication management,
  • Ability to monitor their health, nutrition and physical condition,
  • Ability to self-monitor and self-regulate the above.

 

Employee must exercise considerable initiative and judgment in the effective and efficient administration of the skill building program and collaboration with Tidewater Cove Adult Living Facility. Employee must also exercise tact and courtesy in contact with staff, clients, families, public, and service providers.

Minimum Training and Experience:

MINIMUM QUALIFICATIONS OR STANDARDS REQUIRED

Must be a Licensed Mental Health Professional (physician, licensed clinical psychologist, licensed professional counselor, licensed family therapist, or a psychiatric clinical nurse specialist). Must possess a valid Virginia’s Driver’s License and a driving record acceptable to agency insurance carriers.  Knowledge of DLA 20 assessment. 



Special Requirements:
Must have general knowledge of the principles, practices, and techniques of community behavioral health services, and specific knowledge of Mental Health Skill Building Services.   Required:  training in the DLA 20 Assessment Tool.
