General Information:
Newly formed PACT (Program of Assertive Community Treatment) of the MPNN CSB is seeking a Registered Nurse. This professional ensures the provision of the PACT Model, Assertive Community Treatment and hospital diversion services to adult SMI/COD consumers in the Middle Peninsula Northern Neck Community Services Board (MPNN CSB) area via program coordination, direct service, consultation and education, as well as other intra and interagency functions. Provides general supervision to other nursing staff on the PACT team. Requires managing a number of competing work priorities in a time-restricted environment. Works with a challenging population, often from diverse backgrounds with a variety of concomitant issues. Must be able to work a flexible schedule, including some evenings, weekends, and holidays. Provides services in the community as well as in client homes. Supports the implementation and maintenance of electronic medical records and data collection. This position’s incumbent serves as a Nurse/Case Manager on an interdisciplinary team (PACT). This position works closely with other staff at MPNN CSB using a team approach. Employee must exercise tact and courtesy in contact with staff, clients, families, public, and service providers. 

Minimum Training and Experience:
 Four (4) years of school resulting in the completion of a Registered Nurse program is preferred. Two (2) years as an Registered Nurse. At least one year of clinical experience working with individuals who have been diagnosed with severe mental illness and comorbid general medical conditions. At least one (1) year of case management experience preferred SPECIAL

Special Requirements:
Must possess a valid Virginia Drivers License. Must obtain a TB test with satisfactory results by orientation and annually thereafter as required by licensing authority. Position Open Until Filled. EOE
