General Information:
This employee provides and coordinates substance use assessments, treatment planning, and service delivery tailored to the consumer’s needs.  Ensures the provision of Program of Assertive Community Treatment (PACT) model intensive community treatment and hospital diversion services to adult SMI consumers in the Middle Peninsula Northern Neck Community Services Board (MPNN CSB) service area via care coordination, direct service, consultation and education, as well as other intra and interagency function.  This position’s incumbent serves as a substance use specialist and case manager to a mobile, assertive community treatment team (PACT).  Works with a challenging population, often from diverse backgrounds with a variety of concomitant issues.  Must be able to work a flexible schedule, including some evenings, weekends, and holidays; participation in on-call rotation.  Provides all services in the community as well as in consumer’s homes. This position works closely with other team members using a team approach. Employee must exercise tact and courtesy in contact with staff, clients, families, public, and service providers. 

Minimum Training and Experience:

Bachelor or Master’s degree in human services field (i.e. Psychology, Sociology, Social Work, or Counseling). The PACT Team allows for one QMHP level employee, generally in either the Vocational Specialist or Substance Abuse Specialist position.  One or more years of substance abuse training or supervised experience.  Two or more years’ experience working with individuals with severe mental illness and co-occurring disorders, preferably in a community based treatment model with individuals who have a serious mental illness. Certified Substance Abuse Counselor preferred.


Special Requirements:

Must possess a valid Virginia Driver’s License.  Must obtain a TB test with satisfactory results by orientation and annually thereafter as required by licensing authority.
